To participate in our service, you must have an Ontario Health Insurance Plan (OHIP) or Interim Federal Health Program (IFH). Call our office if you are Uninsured for more information. NO VISITORS (People with valid visitor’s visas). Please print out the registration form, fill it out, and bring it to your first appointment.
